All forms must be completed and returned to your team manager before you or your son/daughter may begin playing during the 2010-11 season.  Forms that must be completed and returned include:

• 2011-12 RG-6 Form
• Medical Release Form
• Player-Parent Contract 
• Publicity Release
• Parental Consent Form
• 2011-12 PS-1 Form
• Player Tryout Form (must be completed prior to any tryout for either a boys or girls team and turned in to team manager.)
